Regarding clinical services –
- The range of clinical services is wide – but not comprehensive. Contact us directly – we can discuss what your needs are over the phone (no charge).
- Labs are collected & run at outside labs (such as Quest Diagnostics) – this can be billed to insurance or we can provide a discounted rate.
- Imaging is done at outside facilities (such as Rayus or Providence Imaging) – this can be billed to insurance or we can provide a discounted rate.
- We do not prescribe long-term opiates (pain management) or benzodiazepines / sedative-hypnotics.
Regarding Fees & Payments –
- Appointment fees are collected at the time of service via cash or card. This fee includes follow-up visits or calls within 30 days. Sliding scale is an option also.
- Lab-work and imaging tests will be done at outside facilities. You can pay the lab or imaging facilities (such as Quest Diagnostics and Rayus Imaging) directly or at a discounted rate through us.
- Not accepting MVA or Workers Compensation cases.
Medicare / Medicaid / Insurance
- We are not currently accepting insurance.
